WHAT IS YOUR NEW PATIENT PROCESS?

We ask that new patients arrive 15 minutes prior to their appointment to fill out required paperwork.

If a patient can fill out their health history form beforehand, ideally at home and on the computer (see Patient Portal), it speeds up the process considerably.

Remember to bring:

-A valid driver's license or other government-issued ID

-Insurance Card

Please understand that copays are due at the time of service.

I DON’T HAVE MY INSURANCE CARD WITH ME. CAN I STILL RECEIVE TREATMENT?

We require your insurance card to check your eligibility. Otherwise, you will be responsible for the procedure/service rendered.

HOW MUCH WILL MY COPAYMENT BE?

We will check your insurance. It varies.
